A Clinical Study on Congestive Heart Failure

Abstract
|
|
|
A clinical observation and analysis were made for 418 cases of congestive´ heart failure from 1976 to 1981.
The results were as follows:
1) In sex distribution, the male was predominant. Male to female ratio was 1.13:1. The incidence was the highest in the 6th decade.
2) The underlying diseases were hypertensive heart disease(42.1%), rheumatic heart disease(34. 0%), postpartum cardiomyopathy(4.1%%), cor pulmonale(3. 1 %), ischemic heart disease(3,1%), pericardial disease(1.9%), thyrotoxic, heart disease(1.7 ), congenital heart disease(1.2%) and others (8.8%). The most frequent- ´underlying disease in male was hypertensive heart disease and that in female was rheumatic heart disease.
3) The common symptoms and signs on admission, were dyspnea(89.0%). pulmonary rale(50.5%), neck vein engorgement(47.3%), peripheral edema (45. 7%), cardiac murmur(41.9%) and hepatomegaly (41.6%).
4) The increased pulse rate over, 100/min . was 32.0%, but less than 60/min was, 2..2%.
5) The laboratory .findings were decreased. total serum protein(45.0.%), decreased serum albumin (43.1%), increased serum globulin(19 9%), abnormal SGOT(43."1%), ,abnormal´ SGPT(24.9%), increased BUN (49.4%), increased serum creatinine (29.5%), albuminuria(61.4%), leukocytosis(41.5%) and increased ESR(27.2%).
6) The chest x-ray on admission revealed increased pulmonary vascularity(73.1%), cardiomegaly (67.3%) . and pleural effusion (20.6%).
7) The EKG findings were abnormal in 93. 0% in 384 cases. The frequent abnormal findings were left ventricular hypertrophy (34.4% and atrial fibrillation (30.5 %) .
8) After treatment, the 156 cases(79.7%) were improved, but the 54 cases(j2.1%) were expired.
